The Drug Formulary is a continually updated list of medications covered for Lipa OHP members. The purpose of the Drug Formulary is to encourage the use of safe, effective, and affordable medications. Users can search for formulary medications by the therapeutic class, or the generic and brand name. If there are restrictions for the use of a specific drug, that information is accessible via a link on the search results. For those drugs that require a prior authorization, please fax a completed Prior Authorization Request Form to 541-302-8052.
